Semaglutide activates the GLP-1 receptor. Tirzepatide activates GLP-1 and GIP. That single difference explains most of what patients notice between the two.
GLP-1 governs appetite signalling, gastric emptying and insulin response. Adding GIP appears to change how the body stores and mobilises fat, which is why average weight reduction in trials has been higher for the dual agonist.
Higher average results do not automatically make tirzepatide the right starting point. Many providers begin with semaglutide because it has the longer safety record, costs less, and produces excellent results for a large share of patients. Moving up is straightforward if progress stalls.
Side effect profiles overlap heavily. Nausea, constipation and reflux are the common complaints for both, and both are managed the same way: slower titration, adequate hydration, smaller meals and enough protein.
The honest summary is that the better medication is the one your provider selects after reading your history, not the one with the stronger headline number.
