These Terms of Use (“Terms”) govern your access to and use of the Longevix MD website and services (the “Services”). By accessing or using the Services, you agree to be bound by these Terms. If you do not agree, do not use the Services.
You must be at least 18 years old and legally able to enter into a binding contract to use the Services. By using the Services, you represent that you meet these requirements and that all information you provide is accurate and current.
Longevix MD is a telehealth platform that connects patients with independent, licensed healthcare providers for evaluation and, where clinically appropriate, treatment. Longevix MD is not itself a healthcare provider and does not practice medicine. Providers exercise independent clinical judgment and are solely responsible for treatment decisions.
You are responsible for maintaining the confidentiality of your account credentials and for all activity under your account. Notify us immediately of any unauthorized use. You agree to provide accurate, current and complete information when creating an account and to keep it updated.
Many treatments are billed on a recurring basis, typically monthly, at the price disclosed to you before purchase. By starting a subscription, you authorize us to charge your payment method on each billing cycle until you cancel.
You may cancel your subscription at any time through your account or by contacting support. Cancellation takes effect at the end of your current billing period unless otherwise stated. We do not charge a cancellation fee.
If a provider determines that treatment is not clinically appropriate for you, any visit fee already paid will be refunded in full. Refunds for medication already shipped are handled case by case and, for prescription products, are subject to applicable pharmacy and safety regulations.
All content on the Services, including text, graphics, logos and software, is owned by Longevix MD or its licensors and is protected by intellectual property law. You may not copy, reproduce or create derivative works from our content without permission.
The Services are provided on an “as is” and “as available” basis. We do not guarantee that treatment will be prescribed, that any treatment will produce a particular result, or that the Services will be uninterrupted or error free. Individual results vary. We make no claim that any compounded product is FDA approved.
To the maximum extent permitted by law, Longevix MD and its affiliates will not be liable for any indirect, incidental, special, consequential or punitive damages, or any loss of profits or revenues, arising from your use of the Services. Our total liability for any claim arising from the Services will not exceed the amount you paid to Longevix MD in the twelve months preceding the claim. Nothing in these Terms limits liability for matters that cannot be limited under applicable law, including gross negligence or willful misconduct by a treating provider.
You agree to indemnify and hold Longevix MD harmless from claims arising out of your breach of these Terms, misuse of the Services, or violation of applicable law.
These Terms are governed by the laws of the state in which Longevix MD is organized, without regard to conflict of law principles, except where applicable state consumer protection or telehealth law requires otherwise for services rendered to you.
We may update these Terms from time to time. Continued use of the Services after changes take effect constitutes acceptance of the updated Terms.
