This Telehealth Informed Consent (“Consent”) describes the nature of telehealth services offered through Longevix MD (“Longevix,” “we,” “us”) and the risks, benefits and limitations associated with receiving care this way. By using Longevix MD to request a consultation, you acknowledge that you have read and understood this Consent and agree to receive care on this basis.
Telehealth involves the delivery of healthcare services using electronic communications, including secure messaging, questionnaires, and, where applicable, audio or video consultation, to enable a licensed provider to evaluate, diagnose, consult with and treat you without an in-person physical examination.
Providers who evaluate your intake are physicians or licensed advanced practice clinicians, licensed in the state where you are physically located at the time of your visit, practicing within the scope of their license.
Telehealth has inherent limitations compared to an in-person examination. Your provider cannot perform a physical exam, palpate, auscultate or directly observe you in person. Information you provide may be incomplete, and diagnostic accuracy can be affected by transmission or technical issues, or by information you are unable or unwilling to share electronically.
Certain conditions cannot be safely or adequately evaluated through telehealth. Your provider may determine that an in-person examination, laboratory testing, imaging or referral to a specialist is required, and may decline to prescribe or continue treatment on that basis.
Submitting an intake or paying a visit fee does not guarantee that any treatment will be prescribed. Every request is independently reviewed by a licensed provider, who will prescribe only when doing so is clinically appropriate based on your history, symptoms and any information available to them. If your provider determines treatment is not appropriate, you will not be charged for medication.
Even when treatment is prescribed, we do not guarantee any specific result. Individual results vary, and no outcome, including weight loss, symptom relief or any other benefit, is promised.
Electronic communications can be interrupted, delayed, or intercepted by unauthorized parties despite reasonable security safeguards. Technical failures may delay evaluation, diagnosis or treatment.
The absence of an in-person exam carries an inherent risk that information necessary for diagnosis and treatment may be missed. You are responsible for providing complete and accurate information about your health history, current medications, allergies and symptoms.
Longevix MD is not an emergency service and providers are not available for emergency response. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use telehealth messaging or wait for a provider response in an emergency.
By proceeding with an online visit, you consent to receive evaluation and, where clinically appropriate, treatment via telehealth from a licensed provider. You confirm that you are the patient described in the intake, or are legally authorized to submit information on that patient’s behalf, and that the information you provide is accurate and complete to the best of your knowledge.
You may withdraw this consent at any time before treatment begins by declining to proceed, and you may stop treatment at any time by contacting your provider or support. Withdrawing consent may affect your ability to continue receiving services through Longevix MD.
Information disclosed during your telehealth visit is generally confidential and protected in the same manner as records from an in-person visit, subject to applicable law and the exceptions described in our Privacy Policy and HIPAA Notice of Privacy Practices.
