HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Longevix MD and the independent licensed providers who treat you through our platform are committed to protecting your protected health information (“PHI”) in accordance with the Health Insurance Portability and Accountability Act (“HIPAA”).

1Our commitment to your privacy

We are required by law to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices regarding PHI, and to notify you following a breach of unsecured PHI. We must abide by the terms of this notice while it is in effect.

2How we may use and disclose your PHI

Treatment

We use and disclose PHI to provide, coordinate and manage your care, including sharing information between your provider and the dispensing pharmacy so your prescription can be filled correctly and safely.

Payment

We use and disclose PHI as needed to obtain payment for services, such as processing your visit fee or subscription charge.

Healthcare operations

We use PHI for activities such as quality assessment, provider review, training and business management necessary to run Longevix MD.

As required or permitted by law

We may disclose PHI without your authorization when required by law, for public health activities, to report suspected abuse or neglect where legally mandated, for health oversight activities, in response to a valid court order or subpoena, or to avert a serious threat to health or safety.

Other uses require your authorization

Any use or disclosure of PHI not described above, including most uses for marketing purposes and any sale of PHI, requires your written authorization, which you may revoke at any time.

3Your rights regarding your PHI

  • The right to request access to and a copy of your medical record
  • The right to request an amendment to your record if you believe it is incomplete or incorrect
  • The right to request an accounting of certain disclosures we have made of your PHI
  • The right to request restrictions on certain uses and disclosures, which we are not always required to agree to
  • The right to request confidential communications, such as being contacted only by a particular method or at a particular address
  • The right to receive a paper copy of this notice upon request
  • The right to be notified in the event of a breach of your unsecured PHI

4How to exercise your rights

To exercise any of the rights described above, submit a written request to support@longevixmd.com. We will respond within the time required by HIPAA, generally 30 days, and may charge a reasonable, cost-based fee for copies where permitted by law.

5Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at support@longevixmd.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.

6Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already maintain, as well as PHI we receive in the future. The current notice will always be available on this page.
